How to make a claim ?

For Planned Admissions

05 Steps

To lodge a claim

Show Health card for claim at Panel Hospital / Facility.
File claim in case of utilizing non panel Facility / Hospital.

Inform us

Inform us 24-48 hours prior to your Admission date.

Claim Form

Download the claim form: click here

Copy of necessary documents to be attached with the form

Health card and CNIC need.
Original Hospital itemized bill with stamp and all original cash receipts
Copies of prescriptions, tests reports, birth/death certificates need to be attached where required.

Acceptance of Claim

Our Medical Team will evaluate your case. We’ll authorize it in line with your policy plan. You will avail the treatment and get discharged from the hospital.

Emergency Admissions

05 Steps

Need for medical treatment in hospital as advised by qualified doctor through OPD/emergency

Visit nearest network / panel hospital for the treatment.

Present Health card and CNIC (copies may be retained by hospital for billing)

Hospital will take approval from Salaam Takaful Health Card through email/phone, as per plan/ entitlement of Patient

5

Sign the bill preferably at the time of discharge and pay the difference if any (room or limit difference)
